Before your first appointment for endodontic treatment, we strongly recommend you take the time in the comfort of your home to fill out your medical history, list of medications you are taking and prepare any questions you might have for our doctors. Our office will be contacting you with the instructions to follow.

If you are unable to complete these tasks at home please bring the list of medications with you. We will be happy to assist and walk you through the collection of your information.
